Hours of operation: Monday – Friday, 8:00 a.m. to 5:00 p.m.
The Center is closed nights, weekends, and holidays

Salary: $101,000.00 – $120,000.00 per year

Responsibilities (include, but not limited to):

  • Performing health histories and exams prior to enrollment and scheduled assessment times thereafter
  • Orders, interprets, and follows up on lab test results
  • Provides education to Participants and their family members as needed
  • Initiates, evaluates, reviews, and changes orders for medications and diagnostic studies/treatments
  • Requests consultations from all interdisciplinary services as needed in collaboration with attending physician
  • Acts as liaison with physicians, nurses, and other healthcare professionals within the program

LIFE-NWPA, a community-based program providing services to seniors allowing them to remain in the community (versus skilled nursing facility), is seeking a Certified Registered Nurse Practitioner for our LIFE Center located in Warren, PA. LIFE (Living Independent for the Elderly), known nationally as PACE (Program of All Inclusive Care for the Elderly) provides support services to its elite group of seniors, enabling them to safely live in their own homes.

Certified Registered Nurse Practitioner, in collaboration with the Medical Director/PCP and fellow interdisciplinary team members, establish and follow the Participant’s Care Plan enabling seniors to remain safely in the community.

Employment Requirements:

  • Successful completion of Pennsylvania Criminal History Check, PA DHS Child Abuse Clearance, Office of Inspector General check, and FBI Fingerprinting for Child Protective Services Law
  • Successful completion of post-offer employment drug screening and physical
  • CPR and First Aid Certification, or willing to obtain at employer’s expense

Education:

  • Master of Science in Nursing
  • Certification of Registered Nurse Practitioner
