Home Care Team Leaders accustomed to the high demands of previous hospital, skilled nursing, group home, and/or community service agencies will enjoy LIFE’s work-life balance, stable 40 hour work week, and a supportive committed and caring team. Occasional overtime opportunities inclusive of weekends are available. Standard operating hours are 8:00 am – 4:30 pm; Monday – Friday. Please note, work hours may fluctuate depending on Participant and business needs, flexibility is required.

Duties include, but are not limited to:

  • Coordinating home care needs of Participants
  • Maintains home care schedules and modify as necessary
  • Completing 6 month assessments for each Participant
  • Perform home visits, including supervisory visits of PCA staff
  • Perform safety assessments of the home environment
  • Documentation in the Participant’s Electronic Medical Record
  • Ordering of durable medical equipment and meals as appropriate
  • Attending daily meetings
  • Participate in Quality Improvement initiatives as necessary

Employment Requirements:

  • Possession of valid Driver’s License and access to own vehicle
  • Successful completion of Pennsylvania Criminal History Check, PA DHS Child Abuse Clearance, Office of Inspector General check, and FBI Fingerprinting for Child Protective Services Law (at employer’s expense)
  • Successful completion of post-offer employment drug screening and physical (at employer’s expense)
  • CPR and First Aid Certification, or willing to obtain at employer’s expense

Salary: $17.00 – $18.00 per hour
