LIFE-NWPA, a community-based program providing services to seniors allowing them to remain in the community (versus skilled nursing facility), is seeking a Primary Care Physician (PCP) for our LIFE Center located in Clarion County, Clarion, Pennsylvania.  LIFE (Living Independent for the Elderly), known nationally as PACE (Program of All Inclusive Care for the Elderly) provides support services to its elite group of seniors, enabling them to safely live in their own homes.

Working within an outpatient clinic, the Primary Care Physician, in collaboration with the Medical Director, Certified Registered Nurse Practitioners, Registered Nurses, and fellow interdisciplinary team members, establish and follow the Participant’s Care Plan enabling seniors to remain safely in the community.

On-site Interdisciplinary Team members include Master Level Social Worker, Registered Dietitian, Physical Therapy, Occupational Therapy, Personal Care Services, In-Home Care Services, Activities, Transportation team, Wellness team, Quality Improvement nurse, and the Center Manager.

Responsibilities (include, but not limited to):

  • Manage care of Participants within an on-site clinic
  • Direct the care of Participants if admitted into skilled nursing facility
  • Performing health histories and physical exams prior to enrollment and scheduled assessment times thereafter
  • Orders, interprets, and follows up on lab test results
  • Provides education to Participants and their family members as needed
  • Initiates, evaluates, reviews, and changes orders for medications and diagnostic studies/treatments
  • Requests consultations from all interdisciplinary services as needed in collaboration with attending physician
  • Acts as liaison with providers, nurses, and other healthcare professionals within the program

Employment Requirements:

  • One year of experience providing care to the frail elderly
  • Successfully complete credentialing process with contracted providers as needed
  • Maintain privileges at two local hospitals
  • Possession of valid Driver’s License and access to own vehicle
  • Successful completion of Pennsylvania Criminal History Check, PA DHS Child Abuse Clearance, Office of Inspector General check, and FBI Fingerprinting for Child Protective Services Law
  • Successful completion of post-offer employment drug screening and physical
  • CPR and First Aid Certification, or willing to obtain at employer’s expense


  • Graduate level education with related physician licensing
