Shift/Hours:  Monday – Friday, 8:00 a.m. to 4:30 p.m.
The Center is closed nights, weekends, and holidays

LIFE-NWPA is seeking a full-time Physical Therapist for our LIFE Center in Clarion, Pennsylvania.  Seniors who chose LIFE-NWPA continue to live in their own homes for as long as possible with the help of an interdisciplinary team instead of having to manage their care by themselves.  LIFE approaches care from a holistic perspective that makes sure that care becomes all-inclusive.

Therapists with LIFE-NWPA enjoy the opportunity to improve the lives of seniors without the constraints of health insurance authorizations and/or limits on time spent with a program participant.

Responsibilities (includes, but not limited to):

  • Evaluating and assessing Participant’s mobility level and other functional abilities
  • Coordination of physical therapy services with the LIFE Interdisciplinary Team and Care Plan Team
  • Conducting initial and ongoing assessments and functional status
  • Develops treatment programs when indicated
  • Sets goals for Participants
  • Provides education to Participant and family as necessary
  • Completes all required documentation of services
  • Performing health histories and exams prior to enrollment and scheduled assessments thereafter

Employment Requirements:

  • One year of experience providing care to the frail elderly
  • Successful completion of Pennsylvania Criminal History Check, PA DHS Child Abuse Clearance, Office of Inspector General check, and FBI Fingerprinting for Child Protective Services Law
  • Successful completion of post-offer employment drug screening and physical
  • CPR and First Aid Certification, or willing to obtain at employer’s expense

  • Bachelor’s degree
