Registered Nurse:
The LIFE-NWPA nurse helps keep you healthy and safe by providing support and guidance on taking your medications, answering your medical questions, providing medical care, and by helping you follow your medical recommendations. Your nursing team is on call for you 24 hours a day, 7 days a week!

Physical Therapist:
Our physical therapy team (PT) helps you physically navigate your home and community safely while providing and teaching you how to use your durable medical equipment. Your PT team works with you to help keep you strong with wellness and therapy, when recommended, to prevent you from falling and to keep you healthy.

Master’s Social Worker:
Our Social Worker provides and helps coordinate mental health and behavioral health services that you may need. He or she is also available any time that you just may need someone to talk to when you have something on your mind that may be bothering you or when you just need a little bit of help to manage day to day stressors or problems.

Primary Care Provider:
The LIFE-NWPA physician and nurse practitioner are right on site in our LIFE-NWPA clinics to make sure that you get the medical care that you need when you need it! They guide your medical care and your medical team to make sure that all of your medical needs are taken care of as quickly as possible with prompt attention and care!

Driver:
The LIFE-NWPA drivers pick you up right at your door! They safely take you to and from all of your medical appointments and to and from the LIFE Center. LIFE-NWPA has its own fleet of vans that are equipped with wheelchair lifts and many other precautions to keep you safe during your rides with us.

Personal Care Attendant:
Our Personal Care Attendants (PCA) are LIFE-NWPA’s direct care staff that provide you with the one on one care that you need. This one on one care may be provided to you in your home and in our day center!

Home Care Coordinator:
The LIFE-NWPA Home Care Coordinator makes sure that you get the help you need in your home to help support you with your light housekeeping, laundry, and personal care needs when medically necessary. She also makes sure that you have what you may need to stay safe in your home!

The Center Manager is responsible for coordinating the Interdisciplinary Team that takes care of you! It is the role of the Center Manager to prompt the team to think “outside of the box” to help keep you safe, healthy, and independent in your own home for as long as possible!

Occupational Therapist:
The Occupational Therapist (OT) provides assessment and interventions to help you develop, recover & maintain activities of daily living such as bathing, dressing & feeding yourself. The OT also provides equipment and training with adaptive devices including bathroom safety equipment.

The LIFE-NWPA dietitian helps you maintain a healthy lifestyle through diet, meal planning and meal preparation. She can also help you keep a healthy weight and supports you in making the right choices for your individual dietary and medical needs. Your dietician may also recommend meals in your home as well as dietary supplements, when needed, to help keep you healthy!
